Donald Trump’s niece has issued a stark warning about the US President’s health, claiming his condition is deteriorating both mentally and physically. Mary Trump, a psychologist and author, said her uncle’s “decaying” health can no longer be overlooked and suggested he is trapped in a “downward spiral”.
Writing in her weekly newsletter, Ms Trump raised fresh concerns about the 80-year-old president amid ongoing speculation surrounding his wellbeing. Referring to what she described as Mr Trump’s erratic conduct, late-night social media activity and the faltering Iran peace deal, she wrote: “He may still have moments when he appears more coherent, but psychologically he’s in a downward spiral. He’s experiencing constant narcissistic injuries, and nothing terrifies Donald more than humiliation.” Her comments come as scrutiny over the president’s health continues to grow during his second term in office. White House spokesman Steven Cheung hit back strongly at the claims, dismissing Mary Trump’s assessment and branding her a “stone-cold loser who doesn’t have a clue about anything”.

Questions about the president’s condition have been fuelled by reports of swollen ankles, bruising on his hands, a rash on his neck and allegations of mental decline.
In late May, Mr Trump attended Walter Reed Medical Centre for his third hospital visit in 13 months, undergoing a routine medical and dental examination.
Following the appointment, the president was declared “fully fit” to serve. Doctors carried out a CT scan, heart imaging, cancer screenings and other preventative assessments involving 22 specialists.
After the three-hour medical visit, Mr Trump said the tests had gone “PERFECTLY”.
He also boasted about achieving full marks in a cognitive assessment, writing: “Unlike other US Presidents, none of whom have ever taken an approved, high difficulty, Cognitive Test, I scored a perfect 30 out of 30, considered ‘extreme intelligence.’
“In fact, this is my fourth such test, all PERFECT or 120 correct answers out of 120 questions asked! It is very rare that anyone gets a Perfect Score, especially when achieved four times in a row.”

The assessment is believed to have been the Montreal Cognitive Assessment, a screening test commonly used to identify dementia and cognitive impairment.
Previous incidents have also attracted attention. In July 2025, Mr Trump was reportedly seen struggling to stay awake while Medicare administrator Dr Mehmet Oz was speaking about healthcare.
The year before, during his hush money trial, he was observed falling asleep with his mouth open while a key witness was giving evidence.
The president has previously attributed recurring bruising on his hands to his daily use of around 325mg of aspirin, which he says he takes to reduce the risk of heart attacks and strokes.
Mr Trump has also faced criticism for a series of public gaffes over the years, including confusing Joe Biden with Barack Obama, mixing up former Republican rival Nikki Haley with Nancy Pelosi, and incorrectly identifying the presidents of Hungary and Turkey.
However, the White House has rejected suggestions that the President is suffering from declining health. Following a recent medical examination at Walter Reed Medical Centre, Mr Trump was declared “fully fit” to serve, while spokesman Steven Cheung dismissed Mary Trump’s claims as baseless.
